Financial Policy

We accept most major medical plans as well as Medicare. Please contact your medical insurance company to make sure we are on your plan before you make your appointment.

Insurance Coverage. As a service to our patients we will bill the insurance carrier(s) for you. However, the patient is ultimately responsible for paying any and all medical expenses incurred.

While we will help in any way possible, your insurance is a contract between you and your insurance provider. Please contact your insurance provider to verify your benefit level before each visit.

Insurance Referrals/Authorization. We will attempt to obtain appropriate insurance referrals prior to the office visit or procedure. However, it is the patient’s responsibility to verify that these referrals/authorizations are in place before services/tests are performed. All patients should be aware that if services or tests are performed without proper authorization, they may be financially responsible for the entire bill. Insurance / Co-payments. All co-payments are due at the time of service.

No Insurance Coverage. For patients with no insurance coverage, We require all new, uninsured patients to pay $250 for a new appointment.
